Please find below all the requisite forms that must be completed prior to your scheduled appointment. Every client is required to fill out both the General In-Take Form and the COVID-19 Waiver.
Following these initial forms, you will only need to complete those specifically relevant to the particular service you have booked. Completing these forms in advance will optimize the efficiency of your appointment. However, if you prefer, you may opt to complete them at the Studio.
For individuals under the age of 18 who are scheduling a waxing or skincare infusion appointment, parental or guardian consent is mandatory. This consent must be signed on-site at the Studio.
