Forms

Below you will find all of the forms required before your appointment. All clients must fill out the General In-Take Form as well as the COVID-19 Waiver. After those, you need only fill out the forms pertaining to the service you will be receiving. Filling these out ahead of time will maximize your appointment time, but you can choose to do this in the Studio if you wish. All minors (under 18) scheduling for a wax or skincare infusion appointment must have parental/guardian consent, and this must be signed in the Studio.

Policies & Procedures

Appointment Scheduling
Rewind Skin Studio is now a MOBILE ONLY operation servicing North Carolina and West Virginia. Depending on your location, choice of services, and how many clients will be receiving services, travel fees may vary. Please call or text to discuss appointments and availability.

A credit card deposit for 50% of the booked services will be REQUIRED at time of booking.

Jet Plasma packages can still be booked. You must set aside availability for every other day for as many appointments as are in your chosen package. It is suggested to find a friend or family member (or multiple people!) to book as well to offset the travel fees this will incur. Rescheduling may not be possible depending on the situation, so please call as soon as possible if you do need to change an appointment block.
 

Tardiness & No-Shows
We kindly ask that if you are running late to your appointment that you please contact us as soon as you can, via text or phone call. Life happens and we will work with you on a case by case basis.

•  If you are 15min late for your scheduled reservation time, we may not be able to provide the full treatment but will perform an
    abbreviated treatment at the full price.
•  If you are more than 15min late, you may be asked to reschedule.
•  If you are more than 15min late 2 separate times, you will be charged a late fee moving forward.
•  If you no-show to an appointment, a no-show fee will be added to your account to be paid at your next service.


Fees are as follows:
•  15min+ late 2 separate times = $20 added to each service you are late to moving forward.
•  No-show = $30 added to your next service.​

Termination of Service
Certain conditions will preclude or end your service:
•  Contagious illness: including colds, flu, r
ashes, etc.
•  Being under the influence of drugs or alcohol.
•  If the client’s needs are beyond the scope of our practice.
•  Requests for sexual or other inappropriate activities. This will not be tolerated and the client will not be rescheduled if this occurs.


Payment Policy
Rewind Skin Studio accepts all major credit cards, Venmo to our business account, and cash. Payment is due upon the conclusion of your service. We do not accept medical insurance or Care Credit.

Manual credit card payments (if you are not using Venmo and your card must be manually entered for whatever reason) incur a 3.5% fee added on to your total.

Cell Phone Policy
Please silence your phone while in the office, so that you may receive the full relaxation benefits of your visit.

Child Policy
Due to the nature of our business and the equipment in the Studio, we ask that you do not bring children under 16 to your appointment for their safety and ours.

Gratuities
While we sincerely appreciate your acknowledgement of a superior service experience, the most generous gratuity you can provide is the referral of your friends and family!

Returns & Refunds
We are committed to ensuring your satisfaction! Please contact us as soon as possible if you are unsatisfied.
 

•  Products: Currently we only stock sample sizes of products for you to purchase and try at home.  As our business grows we
    may add larger stock. For now, full size products are available from GlyMed Plus through our online storefront, or for
    SkinScript RX by placing an order in-person/over the phone. Both companies will deliver directly to your address. Rewind Skin
    Studio, GlyMed Plus, and SkinScript RX do not offer refunds on products. If your product is completely unopened and unused, you
    may bring it to the Studio for shop credit.
•  Services: We do not offer refunds on services or service packages. If you are in any way dissatisfied with the service you received,
    please contact us immediately so we can reach a solution together.

Privacy Policy
We have created this privacy policy to demonstrate our firm commitment to your privacy and the protection of your information.
 
Rewind Skin Studio respects the privacy of our customers and will not release any customer information to third parties. We never trade, sell, rent, or share addresses or any other personal information with anyone. Any information obtained is used to process your purchase or maintain your client file only. All client files are confiden
tial except when subpoenaed by a court of law. The conversations you have with your practitioner will remain confidential. Email and physical addresses are used solely for the purpose of communication between Rewind Skin Studio and customers. We respect your time and attention by controlling the frequency of any mailings.
 
If you would prefer to cease receiving mail or email, simply let us know and we will update your preferences, or you may unsubscribe through your most recent email.
